3. Fill out your Health Profile and email to YouthandVitalityWeightloss@gmail.com
4. Once Your Health Profile is received we will email you the appropriate At Home Weight Loss Program for you that will include the following tools:
Your Food Menu, Youth and Vitality Weight Loss Recipe Book and a Food Journal, Online access to our “tools” (this shouldn’t take more than 2 business days).
5. Schedule your Zoom (or phone, or email if preferred) Personal Program Review with Demi by emailing us at: YouthandVitalityWeightLoss@gmail.com
